Photograph censor paperwork

I have the following as a PDF file:

Does anyone know how the two part would have been used?

How would the document come? As a single sheet or bound in a booklet with perforations to tear out?

Mark,

they came as separate sheets to be placed on the back of photographs for the detailed caption. They are in a two part format for the PDF file I think. I don't know whether you are a member of WAF but there is a great thread about these forms and copyright stamps that were placed on the back of Kriegsberichters photographs.

Thanks a lot for that link!

I think I will be spending a bit of time going through the link. I will probably be investigating getting some stamps made up before too long as well.
