How would the document come? As a single sheet or bound in a booklet with perforations to tear out?

they came as separate sheets to be placed on the back of photographs for the detailed caption. They are in a two part format for the PDF file I think. I don't know whether you are a member of WAF but there is a great thread about these forms and copyright stamps that were placed on the back of Kriegsberichters photographs.
Pour yourself a beer, put a Lilian Harvey & Willy Fritsch CD on and have a great evening reading this.
